SMARTReporter is an app that warns you when your disks are failing. As the name suggests, the application uses S.M.A.R.T.
SMARTReporter doesn't do much more than checking the S.M.A.R.T status and displaying it to you. It does this by constantly running on your Mac's MenuBar. The color of the icon will correspond to the S.M.A.R.T status. A green icon means that nothing is failing, a gray icon means that the status of your disk is unknown and red is the universal color for "not so good". You can change this so that no icon is placed on the MenuBar, though. The app will check your status every 60 minutes, which you can also change.
There is a logging system that displays every time the app checks for your hard drive's S.M.A.R.T status.
One nice thing about this app is that it can carry out actions when your disks fail. For example, it can run another application, pop up an alert, show a notification using Growl or send an e-mail.
