Document Icon Maker automates the task of creating document file icons. All you need to do is add the app icon, specify some identifiers, and export the resulting icon.

The tool allows you to preview the newly-created icon in the Dock. You can export it to a variety of formats, such as ICNS, Icon Sets, Windows ICO, TIFF, and PNG.
